Managing Atopic Dermatitis in Australian Shepherds. Diagnosis and conclusions

On May 8th, 2024, I had the privilege of consulting via tele-expertise with Dr. Carolina Mesquita regarding an intriguing case involving Akira, a female Australian Shepherd, at our clinic.

Patient Overview

Akira presented with notable symptoms of alopecia on her hind legs and bilateral ocular areas. Her guardians reported excessive scratching, which prompted the visit. Akira's diet has been consistent with Harmony dog food since she was a puppy. Vaccinations and preventive care are up to date.

Household Context

Interestingly, Akira shares her home with Symba, a male Australian Shepherd, who experienced similar dermatological issues the previous year. Symba's history indicated alopecia and erythema, first observed in May 2022 and recurring until May 2023.

Clinical Findings

Primary Lesions: None detected.

Secondary Lesions: Notable alopecia on the tarsi, significant erythema, and bilateral ocular alopecia.

General Skin and Coat Condition: Overall, Akira’s coat appeared healthy, though she exhibited persistent scratching.

Differential Diagnosis: The clinical presentation led us to consider atopic dermatitis with pododermatitis as the primary differential diagnosis.

Treatment Plan

Both Akira and Symba were prescribed oclacitinib. Additionally, Symba’s treatment included an antiseptic shampoo and a topical corticosteroid spray. The management plan aimed to alleviate symptoms and improve their quality of life.

Follow-Up and Recommendations

We scheduled a follow-up appointment for May 29th, 2024, to assess the effectiveness of the treatment and determine if Akira is ready for her vaccinations and potentially monoclonal antibody therapy.

This case underscores the importance of comprehensive history-taking and the value of tele-expertise in veterinary practice. By closely monitoring Akira and Symba's progress, we aim to provide tailored care that addresses their specific needs.
